Enrollment into the Firehouse is open to all families. We encourage you to come to tour our facility, pick up an enrollment packet, and meet our staff. All enrollment forms must be completed and turned in on or before the first day your child starts at our school. All children must be up to date with current immunizations and a copy needs to be provided with the enrollment paperwork. We must require a statement from your child's physician clearing your child of any communicable diseases. Any family choosing not to immunize their child, must fill out a Parent Acknowledgement form from the school and a form provided by the Texas Department of State Health Services. To download this form, please visit www.dshs.state.tx.us/immunize. Please allow 2-4 weeks for this form to be processed by TDSHS and then returned to you to be notarized. o Registration - $50.00 for first child and $30.00 for each additional child upon enrollment o Upon enrollment – every child needs to bring 2 of their own mats (kinder mats – one side blue), sleeping blanket for daily use. All kids bring 2 new mats in fall each year. If you enroll midyear you will need to pay $10 for a mat. o Tuition is due on Mondays by 6:30pm. A late charge of $25.00 will be charged if payment is not received after Mondays. If not paid by the end of the second week your child’s position will be forfeited. o A $30.00 supply fee for all children between Toddlers through Schooler’s is due twice a calendar year separate from normal tuition. (April and September) o 18 months and up Classes for Stretch and Grow – Parents Pay $15 before the 1st of each month for each child. Over summer schoolers participate as well. Tuition will be billed to your account. o Preschool to Schoolers have a summer activity fee of $30 for Here Field trips and $60 for Away Field trips due April before summer starts o You will receive one week of vacation per calendar year. This is earned after being enrolled for year. A written notice must be submitted to the Director to get the vacation credit to your account. This must be in writing 30 days prior to using your one week of vacation if you are eligible. o Extra clothes are required in every child’s cubby o Diapers/Pull ups/Wipes are needed for every child that is not potty trained; your child’s daily sheet will let you know when your child needs more of these items. o Drop off time is before 9:30am so all our children will get to participate in the daily activities without disrupting the children in the classroom. o Schooler’s – In-Service/School Holidays - $10.00 per day. o T-Shirts will be used on all field trips, t-shirts will be provided by us for your Schooler child to wear and use. o If withdrawing from our school, you must give a two week notice to the Director in writing to notify us of the drop. If you don’t honor this your account will be charged and billed to you. No copies of your paperwork will be made at your withdrawal. Firehouse has the right to end care at any time without notice. No refund of tuition will be given at the time of dis-enrolment, children terminations and/or parent removal. o End of year tax statements are available before January 31st on all zero-balance tuition. Parents can contact office for your families account tax statement. o The Firehouse Day School reserves the right to change all and any prices at any time
